What is vertebroplasty?

Vertebroplasty, kyphoplasty, and stentoplasty are among the latest methods for treating spinal disorders such as:

  • osteoporotic compression fractures of vertebral bodies,
  • hemangiomas,
  • cancer metastases.

These are minimally invasive procedures that help reduce or eliminate back pain by providing mechanical support inside the vertebra and preventing further progression of spinal deformity. Unlike traditional spinal surgery methods, patients undergoing vertebroplasty, kyphoplasty, or stentoplasty can get up just a few hours after the procedure and return to daily activities pain-free.

Vertebroplasty involves filling the interior of the fractured vertebral body with polymethyl methacrylate (PMMA), known as „bone cement,” or in some cases, another remodeling material.

Kyphoplasty differs from vertebroplasty in that before filling with cement, the vertebra is realigned—restoring vertebral body height by inflating balloons inside the vertebra.

Stentoplasty differs from kyphoplasty in that an implant (stent, e.g., VBS, SpineJack, or others) is placed inside the vertebra simultaneously with realignment, stabilizing the vertebra from within. Only after implant placement is the vertebral body filled with „bone cement.”

Indications for vertebroplasty

These procedures are dedicated to patients with the following spinal conditions:

  • compression fractures or risk of spinal fractures caused by bone weakening due to osteoporosis
  • hemangiomas, i.e., vascular tumors of the vertebral bodies, causing increasing pain symptoms
  • cancer metastases from other organs to the spine. In such cases, before filling the vertebra with „cement,” radiofrequency (RF) ablation of the metastatic lesion is often recommended (RF – RadioFrequency – high-frequency electrosurgery – radio waves).

The postoperative scar is significantly smaller – about 2-3 mm, with no damage to the paraspinal muscles. Due to the minimal size of the surgical field, the risk of infection at the surgical wound site is also low, and pain around the scar is practically absent.

What does the vertebroplasty procedure look like?
During vertebroplasty, no skin incisions are made as in traditional surgical methods. The procedure involves inserting a specialized needle through the skin into the deformed vertebrae and delivering "bone cement" which hardens after about 20 minutes. The procedure is performed under X-ray guidance, allowing the physician to monitor the correct needle placement. It is usually done under local anesthesia.
What does the kyphoplasty procedure look like?
The kyphoplasty procedure is very similar to vertebroplasty. The only difference is that before introducing bone cement into the deformed vertebra, a special balloon is inserted to expand the fractured vertebra, and then bone cement is injected into the expanded vertebral space.
What does the stentoplasty procedure look like?
During the stentoplasty procedure, an implant stabilizing the repositioned/expanded vertebra is additionally inserted, and then bone cement is injected into the expanded vertebral space. In the case of vertebroplasty, when the patient has a vascular tumor, the cement injected into the site of the hemangioma formation releases heat as it hardens, which causes complete coagulation of the hemangioma. This completely eliminates the cause of the disease in the vertebral body, and the weakened vertebral body is further strengthened by the cement penetrating the bone. Both procedures last about 1 hour and provide immediate improvement in the patient's well-being, eliminating or significantly reducing pain symptoms. After the procedure, the patient remains lying down for several hours, then gets up and can return home to their daily activities.
Possible complications
Vertebroplasty, Kyphoplasty, and Stentoplasty are characterized by high effectiveness, and complications are rare. However, as with the treatment of any disease, the possibility of complications must always be considered. Note: Vertebroplasty often does not restore the normal shape of the vertebra but only strengthens and stabilizes the fractured vertebral body.
