What are varicoceles?

The spermatic cord is an anatomical structure that passes through the inguinal canal into the scrotum. The spermatic cord includes the vas deferens, cremaster muscle, testicular artery, pampiniform venous plexus, blood vessels supplying the cremaster muscle, and the genital branch of the genitofemoral nerve.

Varicoceles are pathological dilations of the veins that form the pampiniform venous plexus within the spermatic cord. Varicoceles are considered the most common correctable cause of male infertility.

Causes of varicocele formation

The primary cause of varicocele formation is increased pressure in the veins of the spermatic cord, which results in greater pressure of the flowing blood on the walls of the veins in the pampiniform plexus. This situation leads to changes in the structure of the veins in the pampiniform plexus, causing varicocele formation.

Varicoceles may also result from valve dysfunction or congenital connective tissue disorders. Sometimes, venous thrombosis, collateral circulation, or external compression of the veins caused, among other things, by a tumor, is responsible for varicocele development.

Classification of varicoceles according to their degree of advancement

Based on the degree of advancement, varicoceles are classified as:

Grade I: small varicoceles, not palpable during examination. Noticeable only when standing, especially with abdominal muscle tension.

2

Grade II: varicoceles that can be felt during palpation. They enlarge with abdominal muscle tension.

3

Grade III: large varicoceles, clearly visible regardless of the patient’s position, without the need to tense the abdominal muscles.

Treatment of varicocele

The most effective method of treating varicocele is surgical intervention.

A minimally invasive surgical method for removing such varicoceles is the laparoscopic procedure (laparoscopic varicocelectomy), which involves closing the abnormal blood flow to the testicular veins. The procedure is performed on a day-surgery basis, lasts about 45-60 minutes, and is done under general anesthesia. Through three incisions in the abdominal wall, a laparoscopic camera and instruments are introduced, with which the urologist performs actions to prevent further abnormal blood flow to the spermatic cord, scrotum, and testicle. The patient can return home the same day.

Effectiveness of Varicocele Surgery

Surgical treatment of varicocele eliminates existing symptoms, improves sperm quality and motility, and increases their overall count. The first noticeable improvement in test results takes about 3 months, because that is how long it takes for new sperm to develop. Full effects are expected after about a year. However, sometimes the sperm count does not increase significantly, but their quality improves, which enhances the chances, for example, of success with assisted reproductive techniques.

Varicocele occurs in 15% of men, but in as many as 40% of patients already diagnosed with infertility, making it the most common cause. The problem of infertility affects nearly 15% of couples trying to conceive, with the male factor responsible in 30% of cases.
How does varicocele affect fertility?
Among the numerous theories attempting to explain the link between varicocele and fertility impairment, proposed factors include an increase in scrotal temperature by 1–2°C negatively affecting both testes, increased venous pressure leading to reduced arterial blood flow, decreased testosterone production, accumulation of harmful substances in testicular tissues, and the damaging effect of oxidative stress on developing sperm.
How long after the procedure can improvement in semen analysis be expected?
Since the production of new sperm takes 42–76 days, improvement in semen analysis can be observed no earlier than about 3 months after the procedure.
How should I prepare for the procedure?
As part of the qualification for surgical treatment, each patient should have two general semen analyses performed at least one month apart. (Semen collection should be preceded by 2–5 days of sexual abstinence.) A few days before the procedure, the patient should avoid medications from the groups of anticoagulants, nonsteroidal anti-inflammatory drugs, and refrain from alcohol consumption for at least 24 hours before the procedure. The pubic area should be shaved, and a shower taken on the morning of the procedure. In case of an active infection (e.g., cold), the procedure should be postponed until symptoms resolve.
