Perianal fistula is an inflammatory disease characterized by the formation of a channel between the anus and the skin around the anus. It usually develops as a result of inflammation of the glands located in the rectal wall, which may occur almost unnoticed or as a consequence of a perianal abscess. A small opening forms in the anal area, from which purulent discharge occurs. Anal fistula is a chronic condition requiring long-term treatment and frequent proctologist check-ups.

Self-healing of the fistula is rare, and conservative treatment is usually ineffective. For this reason, perianal fistulas are treated surgically.

Indications for the procedure

The main cause of perianal fistula formation is a bacterial infection of the glands located in the anal canal.

Less common causes may include Crohn’s disease, ulcerative colitis, foreign bodies, complications after surgery performed in the anal area, trauma to the anal region, tumors, radiotherapy of tumors in the perineal and pelvic area, tuberculosis, or actinomycosis.

Diagnostic tests
Proctologists often use transrectal ultrasound examination to assess the disease progression. Transrectal ultrasound is the primary diagnostic test for proctological diseases, i.e., disorders of the anus, rectum, and the final section of the digestive tract. Transrectal ultrasound may be ordered when a patient presents symptoms such as: pain in the anus and surrounding area, constipation and/or diarrhea, fecal incontinence, abdominal pain of unknown origin, presence of blood in the stool
Preparation for the procedure
Preparation for surgical procedure includes performing peripheral blood tests, urine tests, ECG, and others recommended by the physician qualifying the patient for surgery and anesthesia. Additionally, the following guidelines are important: the patient must fast before the procedure, any previously taken medications should be discussed with the doctor, and nursing instructions regarding their discontinuation or alternative administration must be followed.
Description of the surgical procedure
Procedures for removing anal fistulas are most often performed under epidural anesthesia ("to the sacrum"). Depending on the nature of the fistula, it can be completely excised, incised and left open to heal, or partially opened with a rubber band placed inside it. Another treatment technique involves injecting special preparations into the fistula lumen to fill the entire fistula.
Contraindications for a planned procedure
Contraindications for the procedure include: cold or recent viral infections, severe general condition of the patient, especially uncontrolled systemic diseases such as ischemic heart disease (recent myocardial infarction), heart failure, respiratory failure, cardiac arrhythmias, arterial hypertension. However, in most patients, these conditions can be managed with careful treatment and patient cooperation to a state where the surgical procedure can be safely performed. Another contraindication is the lack of necessary test results (including, among others, imaging and laboratory tests recommended by the physician).
Possible complications after the procedure
The following complications may occur after perianal fistula surgery: recurrent abscesses in the anal area; chronic skin infection near the anus; worsened control of gas and stool; fistula recurrence; bleeding after the procedure; infection.
Postoperative care
Treatment of anal fistulas and healing after the procedure is a prolonged process requiring very good cooperation between the surgeon and the patient, as well as frequent wound checks.
