What is a pilonidal cyst?

A pilonidal cyst (pilonidal sinus) is an inflammatory disease of the skin and subcutaneous tissue in the sacrococcygeal area (intergluteal cleft), characterized by a blister formed as a result of enlargement and infection of hair follicles in the sacrococcygeal region, that is, at the lower back above the gluteal cleft. The inflammatory process often develops into an acute condition with abscess formation.

This condition is typical for men aged 20-40 – it occurs four times more frequently in men than in women.

The mechanism of pilonidal cyst formation involves the penetration of one or several hair ends into the subcutaneous tissue. This process may lead to the formation of additional, surrounding fistulas, whose shape and direction depend on hair susceptibility.

Symptoms and complaints of a pilonidal cyst

The causes of pilonidal cyst formation are not unequivocal. It is suspected that factors influencing their development include abundant hair growth in the buttock area, increased sweating, repeated micro-injuries (caused, for example, by wearing tight underwear), and insufficient personal hygiene. The intergluteal cleft is characterized by limited air access, which combined with warmth and moisture promotes bacterial growth and inflammation.

Additionally, obesity and reduced immunity caused by long-term use of immunosuppressive drugs favor the appearance of the lesion.

The main symptoms of a developing pilonidal cyst are:

pain felt in the upper part of the intergluteal cleft
a lump, which may be a subcutaneous thickening or an abscess
discharge of purulent-mucous or purulent content from the cyst opening
low-grade fever or fever
general malaise
local swelling and redness of the skin

Diagnostic tests

Postoperative care
